In accordance with the art. 6, item 1, lit. a REGULATION (EU) 2016/679 OF THE EUROPEAN PARLIAMENT AND OF THE COUNCIL from 27th April 2016 on the protection of individuals in connection with the processing of personal data and on the free movement of such data, I hereby give consent for my personal data included in the application form to be processed.

In accordance with the art. 13 section 1 and paragraph 2 of the General Data Protection Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free ovement of such data, and repealing Directive 95/46 / EC (General Data Protection Regulation GDPR), (Journal of Laws EU L 119, of 4.5.2016, p. 1) I inform that:
